With these payment methods, instead of using your actual credit and debit card numbers when you add your card, a unique Device Account Number is assigned, encrypted and securely stored. And when you make a purchase, the Device Account Number alongside a transaction-specific dynamic security code is used to process your payment. So your actual credit or debit card numbers are never shared by Apple with merchants or transmitted with payment.

Submit a Cardholder Dispute Form for incorrectly posted debit card transactions, including when you may have approved the transaction at the point-of-sale. Before submitting the dispute, you must make a reasonable effort to resolve the dispute with the merchant. The dispute can be one of the following situations:

  • Cardholder does not recognize the charge
  • Billed twice for a single purchase
  • Membership cancellation
  • Merchandise was returned
  • Did not receive merchandise
  • Overcharged for purchase
  • Did not receive credit for transaction
  • Paid by other means
  • Service dispute

Cardholders that need assistance with a dispute, please call 888-918-7322. Your card will remain open while the dispute is processed. Transactions must be reported within 60 days of the statement period in which the transaction occurred.

For transaction(s) involving debit card purchase activity and/or ATM withdrawals not originated by the card holder, please follow these instructions for resolution. Note that transactions must be reported within 60 days of the statement period in which the transaction occurred.

  • Immediately report your card lost/stolen by calling 888-918-7802; if outside of the U.S. 636-722-7111 collect
  • Complete a Debit Card Fraud Affidavit. (A notarized signature is required for all ATM fraud claims.)
  • Return completed affidavit to Card Services at mastercard@lfcu.org, fax to 240-747-3376 or mail to 3535 University Boulevard West, Kensington, MD 20895.
